About

The LIFE Fall Prevention Planning Tool (LIFE Tool) is designed to help older adults reduce their risk of falls by teaching them about fall prevention and allowing them to select activities to make a customized fall prevention plan tailored to their needs. The tool was developed by NORC at the University of Chicago with support from the Centers for Disease Control and Prevention (CDC) and CDC Foundation. The development team used an environmental scan, systematic scoping review of published studies,1 subject matter expert interviews, and focus groups with older adults to create the website. NORC, CDC, and CDC Foundation gathered user testing feedback from internal staff and older adults to improve the LIFE Tool prior to its evaluation. Subsequently, the LIFE Tool was evaluated with a different sample of older adults participating in focus groups and a separate larger sample that used the tool and completed an evaluation survey.

For questions, comments, or feedback about the LIFE Tool please contact Eric Strunz (estrunz@cdcfoundation.org), Associate Director for Noninfectious Disease Programs, at CDC Foundation.

This project, “Fall Prevention Planning Tool Development,” was supported by the Centers for Disease Control and Prevention (CDC) of the U.S. Department of Health and Human Services (HHS) as part of a financial assistance award totaling $500,000 with 100 percent funded by CDC/HHS.
