How It Works
The LIFE Fall-Prevention Planning Tool provides easy-to-read information about risks that may make you more likely to fall. The tool offers simple steps you can select to create a personalized fall prevention plan and reduce your risk of falling.
Modules
The LIFE tool is organized into modules, or sections.
- Each module addresses a different risk factor for falling and offers ideas on how to reduce that risk.
- You can go through these modules in any order and select the ones that are most important to you.
- You do not have to look at all modules in one sitting – you can look at some and then come back later to go through the others. Doing even one to two modules the first time is a great start.
- Each module provides a list of resources in case you want to read more about any topic.
How to create your customized LIFE Fall Prevention Plan
- Each module contains a list of steps you can take that could help you reduce your risk of falls.
- Click the check box next to any step that you want to add to your customized plan.
- You can click as many check boxes as you wish.
- Module 9: Your Customized LIFE Fall Prevention Plan contains a list of the steps you've checked in previous modules. This list is your plan!
- Click the PRINT button in Module 9: Your Customized LIFE Fall Prevention Plan to print or save your plan.
